BOISE, Idaho and MURRIETA, Calif. – January 10, 2018 – Kno2®, the company that provides Kno2: Interoperability as a Service™ (IaaS) to everyone in healthcare, and Care Directives, an organization that makes Advance Directives and Physician-Ordered Life Sustaining Treatment (POLST) documentation available through the cloud with their Advance Directive Information Exchange (ADiE) application, today announced a partnership agreement.

Care Directives is now incorporating Kno2’s industry-leading interoperability service into its cloud-based ADiE solution to enable post-acute care providers and hospitals to send and receive Advance Care Planning (ACP) documents such as Advance Directives and POLST forms via Direct messaging and document query within their normal workflows. Kno2’s interoperability service will ensure that the right information reaches the right provider at the right time while eliminating the need to go into a separate application to check if such documentation exists. As a result, providers at the point of care will be able to make faster, more informed decisions regarding the appropriate level of care to deliver in accordance with the wishes of fragile or terminal patients.

Ensuring timeliness

Care Directives’ ADiE application stores Advance Directives and POLST forms as electronic documents in its database, eliminating the need to fax the documentation on one side and enter it into the EHR on the other. By partnering with Kno2, hospitals and post-acute providers alike can either share documents via Direct messaging or query Care Directives on demand to obtain the latest form. This process makes the information available to the right providers at the right time and ensures that the patient’s most current orders are being followed.

Post-acute care providers can now easily send the latest electronic Advance Directive or POLST forms to the receiving hospital before the patient’s arrival, where it will be incorporated into the EHR automatically, making the information available immediately to clinicians. Should changes be made to the POLST form after the patient is admitted to the hospital, upon discharge the hospital’s Direct messaging will push that information back to Care Directives and the provider currently caring for the patient.
