Kno2 powers the secure transmission of patient information over multiple exchange methods, resulting in significant workflow efficiencies while driving improved care transition and care coordination for users of HCS’s Interactant platform.
HCS’s customers provide care to patients and residents in various care settings including long-term care, long-term acute care hospitals (LTACH), behavioral health, rehabilitation and hospitals. HCS’s integration with Kno2 will assist Interactant users in adoption of interoperable patient document exchange with access to universals directories including Surescripts HISP and the Carequality framework. This integration will facilitate secure digital document transmission, including the exchange of CCDA documentation, while reducing time-intensive paper scanning and traditional fax, transmission methods that can introduce potential security risks and human error.
Integration with Kno2’s platform will allow Interactant users to enhance community relationships with referring hospital partners by improving connectivity and interoperability to help reduce current re-hospitalization percentages and to participate as value partners in bundled payments, value-based purchasing models and accountable care organizations.
